If you would like to open a new Wholesale account, please complete the following form and submit it to us. Please email or fax a photocopy of your tax id / resale number to us to complete the registration. Upon approval of your application, you will receive an email with your Wholesale code.

We cannot activate and send your code until we receive your Tax ID or Resale number.
